A unique collaboration that benefits an entire spectrum of providers

Back in 2019, HG collaborated with Fresh Hope Communities (FHC), a one of a kind relationship where HG were engaged to provide ongoing consulting services to support FHC’s Aged Care Funding Instrument (ACFI) funding for care. 

Over the years, having been engaged in subsequent projects, such as compliance remediation, FHC are familiar with HG’s partnership approach.

The most recent engagement was HG’s AN-ACC funding companion.

HG Consultants undergo specialised training to ensure they accurately identify opportunities for funding uplifts and deliver training to client resources on site to facilitate accurate and timely self-management of AN-ACC requirements.

Health Generation’s field-tested knowledge, processes and tools diagnose funding misalignments and risks in management processes and provide outcome-focus solutions. Their primary goal is to optimise the provider’s funding position to align with strategic aspects of their care business.

HG’s expert AN-ACC funding consultant, Luna Lai, collaborated with Fresh Hope’s onsite team to train them to successfully manage AN-ACC requirements going forward. The program focused on comprehensive and congruent progress notes, behaviour documentation, and orderly preparations for assessors’ visits.

FHC’s Operations Manager Trish Foley commended the HG’s team for their expertise and specifically highlighted Luna Lai’s exceptional contribution. The positive feedback underscores the value of the collaborative approach and the impact on staff skill development.

“Luna was amazing. She did such an amazing job for us. Our team highly benefited from her insights and depth of knowledge,” added Trish.

Trish identified the need to not only optimise funding outcomes but also bolster the skill set of her team. As a result of the engagement, Fresh Hope experienced substantial uplifts in funding, significantly exceeding expectations in some areas. 

When asked about what were the biggest insights they received from the HG’s on-site visit, Trish said their teams learnt a lot about how to prepare for a visit from the accessors. “Knowing how to go about preparing progress notes every day and understanding the nitty-gritties about behaviour documentation are all crucial elements. And I think that’s where they probably got most of their growth from by having health generation on site,” she said.

One of the main driving factors towards this engagement was  to uplift FHC funding strategies. “We were aware that there were a few opportunities to uplift and we wanted to identify them and to give the staff the skills to be able to do it themselves,” adds Trish.
